Minister ordained in Mid-year

Our minister became ordained on May 31st. We paid her SS & Med taxes throughout the year and withheld income tax for her. Do I need to give her two W2's? Am I correct that the as of June 1st, the church's portion of the SS/Med and the housing allowance that the church accepted in the "Resolution" are taxable income to her?
